Below are two forms you will fill out when you visit Tryon Urgent Care.
One is a patient information form that includes your insurance and personal information and the reason for your visit. It also contains a consent to file a claim with your insurance company and the assumption of any remaining charges.
The second is a privacy notice form that describes how your personal and medical information will be used.
